What is the difference between Kinds of Prayers and Forms of Prayers?

To me prayers is supplication (request) made to God. Supplication may be of different kinds, like

1. For repentance;
   
2. For salvation;
   
3. For breakthrough
   
4. For guidance
   
5. For healing
   
6. For miracles
   
7. For personal growth etc.

It is up to an individual, if they want to break up prayers in to different kinds or forms. To me prayer as supplication has no kinds and forms. As Jesus taught us the Lord's Prayer, all the models of prayers work on that. Jesus could have told us that there are different kinds and forms of prayers and even disciples never mentioned kinds and forms.
